• Volleyball (Grades 5 - 8)

• Track (Club Grades 5 - 8)

• Golf (Club Grades 5-8)

Physical Examination IHSA Athletic Physical Form

Each student who intends to participate (including practice) in interscholastic athletics must have a certificate of physical fitness on file in the office before he/she may practice. The physical is good for (395 days). Sixth grade school physicals will work as long as the Doctor checks the box for extracurricular activities.
