Requirements to Participate in Athletics at CHS

Physical Examination IHSA Athletic Physical Form

Each student who intends to participate (including practice) in interscholastic athletics must have a certificate of physical fitness on file in the office before he/she may practice. The physical is good for (395 days). Freshman school physicals will work as long as the Doctor checks the box for extracurricular activities.

  3. Begin Registration (this has several steps, please make sure you complete all steps in the process and hit submit at the end)

Pay the $50.00 Athletic Fee ($50.00/sport for each athlete with a maximum total of $100.00/athlete ... Play 3 get 1 FREE)

Academic Eligibility Requirements to Participate in Athletics
The academic eligibility requirements at Carlyle High School for a student’s participation in any extracurricular activity exceeds the scholastic standing requirements established by the Illinois High School Association. Students must pass thirty (30) credit hours of high school work per week. Generally, thirty (30) credit hours is the equivalent of six (6) “full credit” courses. Students must have passed and received credit toward graduation for thirty (30) credit hours of high school work for the entire previous semester to be eligible the following semester.
